29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. (a) Sawmill: On or about August 29, 2017, employee(s) were changing the teeth on the bottom blade of the Head Saw without the use of lockout device on the electrical energy source. No energy control program, employee training and periodic inspection was established for the maintenance and repair of the Head Saw system at this facility. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.334(a)(2)(i): Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or crushed outer jacket): (a) Sawmill: On or about August 29, 2017 the Stihl electric chain saw, model E14, was not visually inspected prior to use and had damage to the outer sheath jacket exposing the internal insulated conductors. (b) Sawmill: On or about August 29, 2017 the Stihl electric chain saw, model MSE 180C, was not visually inspected prior to use and had damage to the outer sheath jacket exposing the internal insulated conductors. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
