29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about 08/29/2017 - 146 Secretariat Place, Madison, AL, employees were utilizing a fiberglass extension ladder to gain access to the rafter system to install 4'x 8' sheets of plywood over the rafter to establish the substrate that roofing materials would be installed on without having the ladder extended to the minimum 3 feet.
