29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At 4119 Park Crossing Dr., Chelsea, Alabama:  On or about August 29, 2017 and at times prior; the employer allowed his employee to install wrap on a residential home while standing on the roof without fall protection exposing the employee to a fall hazard of 9 feet 10 inches to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At 4119 Park Crossing Dr., Chelsea, Alabama:  On or about August 29, 2017 and at times prior; the employer allowed his employee to install wrap on a residential home while standing on the roof and the employer failed to train the employee on fall protection to include but not limited to how to set up fall protection, the hazards of falls and how to inspect a harness exposing the employee to a fall hazard of 9 feet 10 inches to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a)  At 4119 Park Crossing Dr., Chelsea, Alabama:  On or about August 29, 2017 and at times prior; the employer allowed his employee to use an extension ladder to gain access to the roof top and the rails of the ladder did not extend at least three feet above the landing area to provide safe access exposing the employee to a fall hazard of 9 feet 10 inches to the ground below.
