Main Stage Center Scaffolding. a) Employees were not provided with a safe means of access while working to secure preset diagonal staging/scaffolding sections into rosettes. Employees climbed on the components during the erection process. On about 08/23/2017.

29 CFR 1926.451(g)(3): In addition to meeting the requirements of 1926.502(d), personal fall arrest systems used on scaffolds shall be attached by lanyard to a vertical lifeline, horizontal lifeline, or scaffold structural member. Vertical lifelines shall not be used when overhead components, such as overhead protection or additional platform levels, are part of a single-point or two-point adjustable suspension scaffold. Location: Icahn Stadium 20 Randall's Island, NY, NY. Main Stage Center Scaffolding. a) Employees were attached to the scaffold diagonal pole while working to secure the staging/scaffolding and attached their personal fall arrest system onto the components. . The diagonal piece the employee grabbed into was held by nylon/plastic zip ties, one zip tie snapped and an employee fell approximately 35 feet to the concrete floor. On about 08/23/2017.

Employees were attached to the scaffold while working to secure preset diagonal scaffolding sections into rosettes that were not independent from the scaffold. The diagonal piece the employee grabbed into was held by nylon/plastic zip ties, one zip tie snapped and an employee fell approximately 35 feet to the concrete floor. On about 08/23/2017
