29 CFR 1910.219(i)(2): Revolving surfaces of shaft coupling(s) were not covered by a safety sleeve: On or about August 29, 2017, at the blending tower, employees were working around the blending tower and cleaning the fibers out where there was an unguarded rotating shaft coupling, exposing employees to the hazard of being caught in the rotating shaft.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: On or about August 29, 2017, on line 6, near the colander rollers, the rotating shaft projected more than one half the diameter to approximately 4 to 4.5 inches. Employees walk and work by the protruding shaft exposing them to the hazard of being caught in the rotating shaft.
