29 CFR 1904.32(b)(2)(ii): The annual summary did not include one or more of the following elements: the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log.  a) Norfolk Naval Shipyard, St. Juliens Creek Annex Building 168, Portsmouth, VA 23709: The employer combined the recordable injuries and illnesses, number of employees and total hours worked from a Naval Station Norfolk location and a Richmond, VA location, and others with the establishment at Norfolk Naval Shipyard on the OSHA 300A Summary log or equivalent form.  Abatement documentation is required for this item.
