29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:   a) 7 Goodrich Street, Iselin, NJ 08701. Employees were exposed to eye and face injuries when operating pneumatic nail guns to install new roofing materials while not wearing any eye and or face protection.   Occurred on 08/30/2017.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:  a) 7 Goodrich Street, Iselin, NJ 08701. Employees were exposed to fall hazards in excess of 25 feet from the eave to the ground below when installing new roofing materials while not waring appropriate fall protection. Three of the four employees were wearing belts in place of full body harnesses.   Occurred on 08/30/2017.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) 7 Goodrich Street, Iselin, NJ 08701. Employees were exposed to fall hazards in excess of 25 feet from the eave to the ground below when the portable ladder was not extended three feet above the landing.   Occurred on 08/30/2017.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired:    a) 7 Goodrich Street, Iselin, NJ 08701. Employees were exposed to fall hazards in excess of 25 feet from the eave to the ground below when using a defective portable ladder to access the roof when the ladder should have been marked "Do Not Use" and removed from the jobsite.  Occurred on 08/30/2017.
