29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) shall be restrained from tipping by guying, tying, bracing, or equivalent means: a) Employees were exposed to fall hazard (scaffold tip-over hazard) when standing at the top of the 19 ft tall mobile scaffold. OSHA standard 451(c)(1) allows max height 17.5 ft for a 53 inch wide scaffold. No outrigger bracing or other restraint was provided. Location: 625 W 57th Street, New York, N.Y. On or about 8/30/2017 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) though (vii) establish the types of fall protection to be provided to employees on each type of scaffold. Paragraph (g)(2) addresses fall protection for scaffold erectors and dismantlers: a) Employees were exposed to 19 ft fall hazard working at the top of a rolling scaffold set up on the sidewalk; there were no guardrails or other means of fall protection (harness and lifelines).
