29 CFR 1926.602(b)(1): Operator did not wear seat belt as required for the operators when seated in the normal seating arrangement for equipment operation and or machine operation covered under paragraph (a) of this section:        a) 7 Mason Hill Road, Warren NJ 07059. The employee/operator do not wear the provided seatbelt while operating a John Deere 160G Excavator. Employee suffered head and body injuries when loading the excavator on an uneven flatbed trailer when the excavator slid off the side of the trailer and tipped over.       Occurred on 08/30/2017.
