The employer did not provide refresher training to forklift operators for more than three years after they had been certified to operate a forklift. This condition exposed employees to the hazard of being struck-by materials and equipment involving the improper operation of a forklift.

29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use: (a) Animal Supply Company at 10000 East 56th Avenue, Denver, CO 80238: On and before August 30, 2017, the employer did not maintain a safety data sheet for each hazardous chemical and lubricant, flammable liquid, and combustible liquid stored in the warehouse. These chemicals include Isopropyl Rubbing Alcohol, Premium Plus Interior Eggshell Enamel, Cerema Bryte Stainless Steel Cleaning Polish, Clorox Disinfecting Wipes, Electronic Cleaner, and Dust Destroyer. This condition exposed employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.
