29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about August 30, 2017, at the intersection of SR 80 and County Road 833, North Side, the employer had not provided any type of cave-in protection for two employees that had been sent inside the excavation which was 7'6" in depth, 22' in length, and 18'6" in width, and class C soil, exposing the employee to an engulfment hazard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about August 30, 2017, at the intersection of SR 80 and County Road 833, North Side, the employer had not provided a ladder that extended 3' above the landing of the excavation which was 7'6" in depth, 22' in length, and 18'6" in width, and class C soil, exposing the employee to a fall/engulfment hazard.
