29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:      a) Rubber Department: The work rests on a Safety Seal 4" double sided bench grinder were not adjusted to within 1/8 inch on either side. The measurements of the openings on the right and left sides were 5/8 and 13/16 inches, on or about 8/31/17.      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Facility, Machine Shop: The Victor lathe #1640B was not provided with a guard over the chuck to protect the operator from flying fragments and rotating parts, on or about 8/31/2017.    b) Facility, Machine Shop: The Victor lathe #S206013 was not provided with a guard over the chuck to protect the operator from flying fragments, on or about 8/31/2017.    c) Facility, Machine Shop: Alzmetall Drill press was not provided with a guard to protect the operator from flying fragments and rotating parts, on or about 8/31/2017.     d) Facility, Machine Shop: CB Super Service, radial drill press No. 4E1045, was not provided with a guard to protect the operator from flying fragments and rotating parts, on or about 8/31/2017.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:    a) Rubber Department: A Safety Seal 4" double sided bench grinder did not have tongue guards on either side and the distance between the top of the peripheral member and the wheel measured 1/2 inch on each side, on or about 8/31/17.      NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded:    a) Machine Shop: The light on the Victor Lathe number 1640 did not have a guard to prevent accidental breakage, on or about 8/31/17.    b) Machine Shop: The light on the Clausing Milling machine, serial number Z-73, did not have a guard to prevent accidental breakage, on or about 8/31/17.     c) Machine Shop: The light on the Cincinnati Milling machine did not have a guard to prevent accidental breakage, on or about 8/31/17.    d) Machine Shop: The light on the Trak Milling machine, serial number 012AF11193, did not have a guard to prevent accidental breakage, on or about 8/31/17.
