(a) On or about August 23, 2017 and at times prior thereto, the controlling contractor did not ensure that his sub-contracted employees were protected when accessing a low slope roof and exposed to a 16 foot fall hazard. (b) On or about August 23, 2017 and at times prior thereto, the controlling contractor accessed a low slope roof without the use of fall protection and exposed to a 16 foot fall hazard.
