On or about August 31, 2017, located at Firestone Complete Auto Care in Canal Winchester, Ohio, an employee was performing painting operations from an unprotected edge of a 3 on 12 pitch roof without any means of fall protection, thereby exposing the employee to a fall hazard of approximately 18 feet.
