(a)  The employer failed to provide documentation to the Agency demonstrating that abatement is completed for the serious violation that was issued by the Agency.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    (a)  Employees conducting roofing activities on a roof with a 4:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 15.6 feet.  The RLP Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1171905, citation number 1, item number 2 and was affirmed as a final order on September 20, 2016, with respect to a workplace located at 623 E. Monroe Ave., Hartford, WI 53027.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  The employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.     The RLP Contractors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1171905, citation number 1, item number 3 and was affirmed as a final order on September 20, 2016, with respect to a workplace located at 623 E.
