29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about August 30, 2017, at a worksite located at 1761 Beall Ave., Wooster, OH 44691, during commercial construction activities the employer did not ensure the operator of the rough terrain forklift, Skytrak F6K-002 was competent and trained. The operator was observed with the load engaging means raised while the rough terrain forklift was unattended.
