29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On or about August 31, 2017, on the 6:12 pitch roof, three roofing employees installing roofing materials were exposed to 8 to 19 feet fall hazards from the eaves and raked edges of gables in that they were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet: a. On or about August 31, 2017, the roof access ladder used by three roofing employees extended only 29 inches above the upper landing surface at the 7 feet and 9 inches high eave.
