On or about August 31, 2017 and at times prior thereto, an employee working from the derrickman's platform was not provided with a safe auxiliary means of egress (Geronimo Line). Employees were exposed to a fall hazard of more than 4 feet in the event of a fire or explosion.

Employees were exposed to a fall from elevation of more than 4 feet while performing routine work.
