29 CFR 1910.147(c)(1): The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable. (a) Facility - On or about August 31, 2017 employer does not have a written energy control program that has equipment listed, specific lock out procedures to deenergize the equipment, person/s responsible for conducting periodic inspections and certification of training of affected and authorized employees that is related to their specific machinery.

29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material shall be such as to permit easy handling of material without the operator placing a hand in the danger zone. Such tools shall not be in lieu of other guarding required by this section, but can only be used to supplement protection provided. (a) Facility - On or about August 31, 2017 employees are exposed to struck-by hazard when removing lint build up from rotating delinter saws.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.
