29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. The employer failed to report to OSHA a work-related hospitalization within the 24 hour time frame, where a hospitalization occurred on August 22, 2017, at approximately 7:30am and was not reported to OSHA until August 23, 2017 at approximately 11:45am.
