29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:      a) Daniel's Roofing and Siding on August 30, 2017, at the address of 28546 Starbright Blvd, Perrysburg, OH: The employer had not trained all employees in the appropriate use of fall protection, such as but not limited to, the use of personal fall arrest systems while performing various roofing operations.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower level was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.      a) Daniel's Roofing and Siding on or about August 30, 2017 at the address of 28546 Starbright Blvd, Perrysburg, Ohio, 43551: The employer did not provide two employees applying new roofing material (asphalt shingles) to a 5/12 pitch commercial structure with fall protection. Employees were exposed to a 7 foot fall onto the level below.
