29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury: On or about August 31, 2017 at the above addressed jobsite, employees were using pneumatic nail guns and were not wearing safety glasses, thereby exposing the employees to the hazards associated with eye injuries. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

Employees were not utilizing fall protection and were exposed to a fall hazard of approximately 11.5 feet to approximately 21.1 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access. On or about August 31, 2017 at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three feet above the upper landing surface, thereby exposing employees to hazards associated with falls. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
