Waste Water Treatment, Agat-Santa Rita construction site:  A rebar bending machine was available for use at the jobsite was energized with missing guards.  Employees were exposed to amputation hazards.

29 CFR 1926.702(j)(1): Employees were permitted to perform maintenance or repair activity on rebar bending equipment used for construction activities where the accidental start-up of the equipment was not eliminated by use of lock out/tag out procedures:  Waste Water Treatment, Agat-Santa Rita construction site: trouble shooting/servicing of the rebar bending machine at the point of operation was performed without applying lock out/tag out procedures. Employee was exposed to the amputation hazards.
