29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Roof, Front Elevation - On or about September 1, 2017, an employee performing painting work while on the roof of a new two-story residence was exposed to fall hazards of up to approximately 13 feet onto earthen ground in that the worker was not fall protected.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    a) Rear Porch - On or about September 1, 2017 and times prior to, a concrete stairway system consisting of 6 risers  was not equipped with a handrail or stair rail system on either of its opens sides, thus exposing employees of fall hazards of up to about 4 feet onto earthen ground.    b) Front Porch - On or about September 1, 2017 and times prior to, a wooden stairway system consisting of 6 risers (including the porch landing) was not equipped with a stair rail system on either of its open sides, thus exposing employees of fall hazards of up to about 4 feet onto earthen ground.    c) Garage - On or about September 1, 2017 and times prior to, a wooden stairway system consisting of 5 risers was not equipped with a stair rail system on either of its open sides, thus exposing employees of fall hazards of up to about 3 feet onto earthen ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Roof, Front Elevation - On or about September 1, 2017, an employee used an aluminum extension ladder to gain access to the roof of the building without the side rails of the ladder being extended at least 3 feet above the upper landing surface, thus exposing employees to fall hazards of up to approximately 13 feet onto earthen ground.
