29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6): a- 19 & 21 Bank Street, Princeton NJ: On or about 08/31/17, a company worker was exposed to electrocution hazards when working from a single board that was set on the rung of an aluminum extension ladder and an adjacent pump jack scaffold's walking/working surface. When working from this single board this exposed worker was working next to/near a live electrical line. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

When working from this single board this exposed worker was not supplied with any fall protection and was working next to/near a live electrical line.
