29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A) Exterior, In bucket truck:  An employee was exposed to head injuries while working in an area with overhead hazards. The employee was not protected by a hard hat or protective helmet, on or about July 26th, 2017.

A) Exterior, In lift: Employees were exposed to electrocution hazards while working in an aerial lift approximately 1 foot away from energized, un-insulated primary power lines, on or about July 26th, 2017.
