29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: A) Worksite - On or about September 1, 2017, an employee was removing dirt from the top of a sewer pipe, and was not provided with a safe means of egress from the excavation, such as a ladder. The excavation was approximately 7.5 feet deep.

The asphalt pavement on the shoulder side of the excavation had been undermined and was not supported.
