29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: a) Controlled Molding, Inc. - employer had not obtained audiograms for employees exposed to noise above the action level. On or about October 11, 2017, the following noise exposures were measured: 1. The clean room manager was exposed to a mixed noise level of 87.4 dBA or an equivalent dose of 70%, as an 8-hour TWA, while conducting scalpel blade assembly operations. This level was based on 445 minutes of sampling; zero exposure was considered for the unsampled time.
