29 CFR 1910.28(b)(15)(iii): Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure each employee on a walking-working surface 4 feet (1.2m) or more above a lower level was protected from falling by a personal fall protection systems, such as personal fall arrest, travel restraint, or work positioning systems:    JOBSITE:  The employer did not ensure through providing specific instructions, visual observations and/or other means that each employee secured the work positioning system while ascending/descending the tree.    On or about 08/29/2017, a branch of the Sycamore tree broke when an employee grabbed the branch while trying to climb the tree, causing the employee to fall approximately 8 feet to the pavement below.  The employee sustained a broken rib, and fractured both of his wrists from the incident.
