29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: (a) The employer did not ensure that employees were protected from a fall hazard of approximately twenty-five (25) feet when an employee was observed working on the framing of a two story house being built at 3005 Deansbrook Ave, Plano, TX and the platforms and walkways on site built scaffolds used by that employee were only approximately ten (10) inches wide.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) The employer did not ensure that each employee framing a roof of a two story house being constructed at 3005 Deansbrook Ave, Plano, TX were protected from a fall hazard in that one employee was working off of site built scaffolding approximately twenty-five (25) feet above the concrete floor below. The scaffolding did not have a guardrail system and/or the employee was not using a personal fall arrest system.
