29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.   On 8/30/17, at the job site 2038 Duluth Hwy., Lawrenceville, Georgia , employees working from a scaffold were not provided a ladder to working levels.

On 8/30/17, at the job site 2038 Duluth Hwy., Lawrenceville, Georgia the employer did not ensure scaffolds used on site by employees were inspected by a competent person prior to being used.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On 8/30/17, at the job site 2038 Duluth Hwy., Lawrenceville, Georgia employees were accessing the low slope roof as a means of access to the inner roof parapet area for brick work and were not protected from falls as they walked up the roof to the area of work.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support.  On 8/30/17, at the job site 2038 Duluth Hwy., Lawrenceville, Georgia, employees were exposed to fall hazards when they accessed roof levels using ladders that were not secured at the top.
