29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying exists.  (a) 1315 Sheridan LLC located at 1315 Sheridan Blvd, Lakewood, CO 80214:  The exposing and controlling employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  Sub-contractors on site were utilizing open sided floors to access scaffolding and other areas within the building.  The staircases used for access did not have hand rails installed, and the scaffolding used for stucco work was not properly configured.  This exposed employees to falls from height.

Employees were using the stairwells to access the upper levels to perform work throughout the project without at least one handrail.  This exposed employees to falls from height.

Employees were working on upper levels without toeboards with other employees working on ground level that were not utilizing head protection.  This exposed employees to struck-by hazards from falling tools and materials.

A stucco subcontractor was using a fabricated frame scaffold approximately 26 feet high for stucco work without the use of adequate guardrails, and no alternative means of fall protection was in use.  This condition exposed employees to falls from height.

29 CFR 1926.501(b)(13): Each employees engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   (a)  1315 Sheridan LLC located at 1315 Sheridan Blvd, Lakewood, CO 80214:  The exposing and controlling employer did not ensure that employees engaged in residential construction activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measures.  Employees were walking and working on the second and third level stairs, walkways, and balconies which did not have guardrails, and without other means of fall protection.  This condition exposed employees to falls from height.  Abatement note:  When used as a means of fall protection, guardrail systems are required to conform to 29 CFR 1926.502(b).  This standard requires, among other things,  that a top rail be located 42 inches high, plus or minus 3 inches, and a mid rail be located at a height halfway between the top edge of the guardrail system and the walking/working level, at least 21 inches high.  The top rail shall support a force of at least 200 pounds applied in any outward or downward direction, and the mid rail shall support a force of at least 150 pounds applied in any outward or downward direction.
