Employee(s) were exposed to fall hazards of up to 108 inches.

29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee:      Building 25024, Andersen AFB: The pipe mounting bracket used to anchor the lanyards was not evaluated to determine if it supported at least 5,000 pounds per employee or maintained a safety factor of at least two. Employees were exposed to fall hazards of up to 108 inches.
