OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a potential catastrophic fire and/or explosion of a gas-fired batch oven. 1) On or about August 18th, 2017 and at times prior, the employer did not ensure that employees were protected from being hit by the explosion-relief vents, when activated. 2) On or about August 18th, 2017, and at times prior, the employer did not establish regularly scheduled inspections, testing, and maintenance of all oven safety devices. 3) On or about August 18th, 2017, and at times prior, the employer did not ensure that the ovens had more than one gas safety shut off valve, and that they were piped in series. 4) On or about August 18th, 2017, and at times prior, the employer did not develop and implement a schedule for making tightness checks on all gas safety shutoff valves.
