29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: On or about 9/5/17 in the Bay 1 Area, the employer allowed employees to use the stairs (12 risers, 40" wide) to access the mezzanine to store equipment and supplies, and the stairs lacked handrails on the open side; thus exposing employees to injuries from trip and falls from elevations.

On or about 9/5/17 in the Bay 1 Area, the employer allowed employees to store equipment and supplies in the mezzanine area located above shop offices without having displayed weight-limit signs; thus exposing employees to hazards of excess weight above their heads.
