(Front of  house) Employee was exposed to fall hazards of f 12 foot, 3 inches from the eave to the ground below while installing new roofing materials when not wearing any fall protection.    Incident occurred on 09/05/17    b) 1292 Amherst Avenue, Union, NJ 07083. (Rear of house) Employees were exposed to fall hazards of 19 foot, 10 inches from the eave to the ground below while installing new roofing materials when not wearing any fall protection. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  a) 1292 Amherst Avenue, Union, NJ 07083. (Rear of house) Employees were exposed to fall hazards of 19 foot, 10 inches from the eave to the ground below while using a Werner portable extension ladder to access the roof when the ladder did not extend three feet above the landing.    Occurred on 09/05/17

(Rear of house) Employees were exposed to fall hazards of 19 foot, 10 inches from the eave to the ground below while using a Werner portable extension ladder to access the roof when the ladder was damaged and not marked Do Not Use and taken out of service.    Occurred on 09/05/17
