29 CFR 1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millenium Edition, December 2000, FHWA:    (a)  Rago Concrete, at 13th Ave. and Fox Street, Denver, CO 80204:  On and before September 1, 2017, the exposing employer did not ensure that traffic control signs or devices conformed with Part VI of the MUTCD.  Flaggers were directing traffic on westbound 13th Ave. to assist cement trucks, who were arriving on 13th Ave., and then backing into Fox Street.  The employer did not ensure that that advanced warning signs were in place on 13th Ave.  This condition exposed the flaggers to the hazard of being struck-by oncoming traffic.
