Hoge Lumber Co., Inc. - New Knoxville, OH: On or about July 31, 2017, the employer did not develop, document, and utilize energy control procedures for authorized employees who clean and service the Brandt KD69 edgebander machine. Employees were exposed to injury in that energy control was not utilized for servicing and maintenance tasks.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees.    a.  Hoge Lumber Co., Inc. - New Knoxville, OH: . On or about July 31, 2017, the employer did not ensure that employees who service and maintain the Brandt KD69 edgebander machine were adequately trained in the skills required for the safe application, usage, and removal of energy controls.  Employees that perform tasks such as cleaning and setup of the equipment were not trained to control the machine's hazardous energy.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6).    a. Hoge Lumber Co., Inc. - New Knoxville, OH: On or about July 31, 2017, employees that cleaned the Brandt KD69 edgebander machine were exposed to pinch point and rotating part hazards.  An employee sustained a severe laceration injury when cleaning the machine parts at a time the machine was not shut down and locked out.  The employer failed to ensure that energy control application steps to control the machine's electrical and pneumatic energy sources were implemented prior to cleaning the machine - the employer did not:   -  Ensure employees had the method or means to control hazardous energy.  -  Ensure that equipment was turned off or shut down.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Verify that isolation and deenergizeation of the machine or equipment had been accomplished.    b. Hoge Lumber Co., Inc. - New Knoxville, OH: On or about September 6, 2017, employees were exposed to pinch point and  rotating part hazards in that the Brandt KD69 edgebander machine was not locked out prior to setting up and making adjustments to the machine. The employer did not ensure that the energy control application steps to control the machine's electrical and pneumatic energy sources were implemented prior to the servicing task - the employer did not:  -  Ensure employees had the method or means to control hazardous energy.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Verify that isolation and deenergizeation of the machine or equipment had been accomplished.    c. Hoge Lumber Co., Inc. - New Knoxville, OH: On or August 23, 2017, employees were exposed to pinch point and  rotating part hazards in that the Brandt KD69 edgebander machine was not locked out prior to repairing a drive chain.   The employer did not ensure that the energy control application steps to control the machine's electrical and pneumatic energy sources were implemented prior to the maintenance task - the employer did not:  -  Affix lockout device(s) to the energy isolating device.  -  Verify that isolation and deenergizeation of the machine or equipment had been accomplished.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.    a. Hoge Lumber Co., Inc. - New Knoxville, OH: On or about July 31, 2017, the employer did not ensure that the Brandt KD69 (s/n 0-261-03-5880) edgebander machine was adequately guarded to protect employees from rotating part hazards.  Employees were exposed to being caught in unguarded rotating parts and nip points near the glue pot and buffing wheels, and employees were not prevented from accessing the hazard area behind the hood at a time the machine could operate.
