(a)E & B Construction, Inc. at 900 East 20th Street, Sioux Falls, SD 57105: On and prior to August 30, 2017, the controlling employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Subcontractor employees were permitted to work in areas where fall hazards existed without the use of a fall protection system. The condition exposed subcontractor employees to a fall hazard of approximately 20-feet.
