29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the east and south side of the construction site, employees performing framing activities without fall protection were exposed to fall hazards of approximately 45 feet to the ground below.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On the east side of the construction site, employees installing windows from a wooden trash bend were elevated by JLG lift that was not approved by the forklift manufacturer and exposed to falls, struck-by, and crushed-by hazards.
