29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (a) Village Park of Alpharetta, Southwest side: On or about 09/06/2017, and at times prior, the employer exposed an employee to fall hazards of approximately 18-feet from the aerial lift platform, in that mid rails were not in place as they were tied to the top rail which exposed an employee engaged in caulking activities between the second and third levels.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Village Park of Alpharetta, Southwest side: On or about 09/07/2017, and at times prior, the employer exposed employees to ejection/fall hazards of 18-feet from the aerial lift basket, in that body restraint equipment was not provided while engaged in caulking and painting activities.
