Additional counterweighting of fork trucks shall not be done unless approved by the truck manufacturer:   On or about a 7/26/17, the employer failed to ensure that employees were not permitted to ride on the back of the Toyota Forklift, Model #8FGU32, to act as a counterweight. When the Toyota forklift became overloaded, and/or driven into a pothole, an employee rode on the back to act as a counterweight to prevent tipping. Counterweighting was not approved by the manufacturer.
