29 CFR 1926.502(i)(2): All other covers shall be capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. (a) Brinkman Construction, at 821 W Lake Street, Fort Collins, CO: On and preceding 8/30/17, the controlling employer did not ensure all covers were capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. Employees performed construction tasks in support of the Stadium Apartments project. An employee stepped on a plywood mechanical well cover and fell 12 feet into the mechanical well. This condition exposed employees to a fall hazard.

(a) Brinkman Construction, at 821 W Lake Street, Fort Collins, CO: On and preceding 8/30/17, the controlling employer did not ensure all covers were secured when installed so as to prevent accidental displacement by the wind, equipment, or employees. Employees performed construction tasks in support of the Stadium Apartments project. An employee stepped on a plywood mechanical well cover, displacing it, and fell 12 feet into the mechanical well. This condition exposed employees to a fall hazard.
