29 CFR 1910.1000(a)(2): Other substances -- 8-hour Time Weighted Averages. An employee's exposure to any substance in Table Z-1, the exposure limit of which is not preceded by a "C", shall not exceed the 8-hour Time Weighted Average given for that substance any 8-hour work shift of a 40-hour work week.     a) On 9/5/17, a Line M2 Machine Operator was exposed to 397% of the 8 hour Time Weighted Average Carbon Monoxide exposure limit. The 8-hour Time Weighted Average exposure limit for Carbon Monoxide is 50 ppm. The Line M2 Machine Operator was exposed to a Carbon Monoxide 8-hour Time Weighted Average of 198.6 ppm over a 445 minute time period.    b) On 9/5/17, a Line M3 Product Inspector was exposed to 316% of the 8 hour Time Weighted Average Carbon Monoxide exposure limit. The 8-hour Time Weighted Average exposure limit for Carbon Monoxide is 50 ppm. The Line M3 Product Inspector was exposed to a Carbon Monoxide 8-hour Time Weighted Average of 157.8 ppm over a 415 minute time period.    c) On 9/5/17, a Line M3 VCS Operator was exposed to 293% of the 8 hour Time Weighted Average Carbon Monoxide exposure limit. The 8-hour Time Weighted Average exposure limit for Carbon Monoxide is 50 ppm. The Line M3 VCS Operator was exposed to a Carbon Monoxide 8-hour Time Weighted Average of 146.6 ppm over a 445 minute time period.    d) On 9/5/17, a Line M3 Case Sealer was exposed to 260% of the 8 hour Time Weighted Average Carbon Monoxide exposure limit. The 8-hour Time Weighted Average exposure limit for Carbon Monoxide is 50 ppm. The Line M3 Case Sealer was exposed to a Carbon Monoxide 8-hour Time Weighted Average of 129.8 ppm over a 445 minute time period.

29 CFR 1910.1000(e): To achieve compliance with paragraphs (a) through (d) of this section, administrative or engineering controls must first be determined and implemented whenever feasible. When such controls are not feasible to achieve full compliance, protective equipment or any other protective measures shall be used to keep the exposure of employees to air contaminants within the limits prescribed in this section. Any equipment and/or technical measures used for this purpose must be approved for each particular use by a competent industrial hygienist or other technically qualified person. Whenever respirators are used, their use shall comply with 1910.134.  a) On or about 9/5/17, the M3 Fryer Burner Section ventilation system was not rebalanced to provide the additional fresh air needed and/or control the additional carbon monoxide generated when 40 of the 100 burner units were replaced on the M3 Line natural gas fueled Fryer Unit during the previous 3 day weekend.  b) On or about 9/5/17, Supervisors did not remove employees as an administrative control, from the M2 and M3 Production Lines Area when employees were experiencing severe headaches, nausea and other symptoms.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.    a) On or about 9/5/17, the employer's hazard communication training did not include the potential of carbon monoxide exposure from the natural gas fueled equipment.    b) On or about 9/5/17,  the employer's hazard communication training did not include training natural gas fuel equipment operators to report abnormal burning conditions, such as flame color or flame height, to supervisors and/or maintenance for repairs.
