29 CFR 1910.95(g)(5)(i): The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared: a.) An employee, edger operator, was exposed to continuous noise at 135% of the exposure level of 80 dBA or a time- weighted average of 102.4 dBA. This exposure was observed over a 116 minute sampling period. Zero exposure was assumed for the time not sampled. The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared, on or about 9-21-17. b.) An employee, trim saw operator, was exposed to continuous noise at 81.2% of the exposure level of 80 dBA or a time- weighted average of 98.7 dBA. This exposure was observed over a 118 minute sampling period. Zero exposure was assumed for the time not sampled. The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared, on or about 9-21-17. c.) An employee, sawyer operator, was exposed to continuous noise at 93.4% of the exposure level of 80 dBA or a time- weighted average of 99.7 dBA. This exposure was observed over a 117 minute sampling period. Zero exposure was assumed for the time not sampled. The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms can be compared, on or about 9-21-17. The employer did not institute a training program and ensure employee participation in the program: a.) The trim saw operator was exposed to noise at or above an 8-hour time-weighted average of 85 decibels and the employer did not train each employee in accordance with the requirements of 29 CFR 1910.95(k), on or about 9-21-17. b.) The edger operator was exposed to noise at or above an 8-hour time-weighted average of 85 decibels and the employer did not train each employee in accordance with the requirements of 29 CFR 1910.95(k), on or about 9-21-17. c.) The sawyer operator was exposed to noise at or above an 8-hour time-weighted average of 85 decibels and the employer did not train each employee in accordance with the requirements of 29 CFR 1910.95(k), on or about 9-21-17.
