29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift. Worksite: The employee, operating the Genie Z45-25, did not have the leg straps of his PFAS secured to his legs nor did have have his lanyard connected to the anchor point on the lift basket.

29 CFR 1910.140(d)(1)(iv): In addition to the general requirements in paragraph (c) of this section, the employer must ensure that personal fall arrest systems:(iv) Sustain the employee within the system/strap configuration without making contact with the employee's neck and chin area.
