29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  On or about August 16, 2017 an employee engaged in lowering a dump bed of an asphalt truck was injured by the dump bed, which resulted in broken ribs and an in-patient hospitalization.  The employer did not report the in-patient hospitalization with in 24 hours as required by 29 CFR 1904.39 (a)(2).   There is no abatement certification or documentation required for this item.
