29 CFR 1926.302(b)(2): Safety clips or retainers were not securely installed or maintained on pneumatic impact (percussion) tools to prevent attachments from being accidentally expelled: a) On or out September 6th, 2017, employee(s) were not protected from struck-by hazards while operating a pneumatic American Pneumatic Tool Co. (APT) paving breaker. The paving breaker (jackhammer) was not secured to the air hose by means of a safety pins in order to prevent accidental disconnection.
