29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating, or moving parts of equipment exposed to contact by the employees or which otherwise created a hazard, were not guarded:  a.  Great Rivers at Tribble Mill Subdivision, 2018 Great Shoals Circle, Lot 64C, Lawrenceville, GA - Employees were exposed to caught-between hazard.  Employees were operating the portable concrete mixer without having the guard covering the rotating parts (drive pulley and belt). Employees were using the concrete mixer for the brick laying process of the newly constructed residential two story home, on or about 09/07/17.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:  a.  Great Rivers at Tribble Mill Subdivision, 2018 Great Shoals Circle, Lot #64C,  Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard.  Employees were working from a tubular welded frame scaffold, installing bricks to the front exterior side of the newly constructed residential home.  The working levels which the employees were working from was not fully planked, creating a fall hazard of approximately 7 feet above ground level, on or about 09/07/17.

29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used for access:  a.  Great Rivers at Tribble Mill Subdivision, 2018 Great Shoals Circle, Lot #64C, Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard.  Employees were climbing the frame of the  two buck high tubular welded frame scaffold to reach the working level platforms.  The employees were not utilizing the proper access ladder which was installed on the scaffold, on or about 09/07/17.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section:   a.  Great Rivers at Tribble Mill Subdivision, 2018 Great Shoals Circle, Lot 64C, Lawrenceville, GA (Jobsite) - Employees were exposed to fall hazard.   Employees were in the process of laying bricks and mortar on the front of the exterior home while standing and walking at the edge of the 2-buck high scaffold platform, approximately 14 feet from ground level, which did not have guard-rails and/or means of a protective system on the walking and working area of the scaffold, on or about 09/07/17.
