29 CFR 1910.110(e)(4)(iii): .LP gas container(s) installed on internal combustion engine vehicles were not securely mounted to prevent jarring loose, slipping or rotating:    a) Facility:  The LP gas cylinder located on the back side of the Hyster 50 series sit down type (fork lift) power industrial truck was not properly secured.  The bracket in the middle of the cylinder was not clipped in during its use as observed on 09/07/2017.

29 CFR 1910.303(g)(1)(ii): Working space provided about electrical equipment rated 600 volts, nominal, or less was used for storage:.  a) Facility: Electrical panel boxes (voltage 480 Volts/3 phase) were not maintained free of storage material, e.g., boxes of beverage products.  A minimum 30 inches of space was not maintained in front of the electrical panels as observed on 9/7/2017.
